New Trends in Refractive Cataract Surgery

People suffering from Cataract can now look forward to a normal eyesight post surgery in most cases thanks to the technological advancement that has taken place in the field of ophthalmology. Phaco-emulsification offers patients a life free from glasses after cataract surgery.

A lot of development has taken place as far as material and design of the IOL is concerned. This has helped in getting better visual outcomes for the cataract patients. The IOLs used extensively so far have been monofocal IOLs. This implies that the power of the lens inserted in eye is such that one can see distance without glasses but near add is required for performing any work at near. This can be incapacitating for many. A long felt requirement has been for an IOL that can be focused for distance as well as near which will actually offer patients to see clearly at all distances. Moreover the common monofocal IOLs also are similar to the older natural lens of the eye which fails compensate for the aberration due to the cornea as the young natural lens.

The current popular trend for cataract surgery worldwide involves the procedure of micro phaco-emulsification with the implantation of a 2nd generation multifocal foldable IOLs. The multifocal lens offers the patient a clear vision across all distances and it is even better if it is a prolate lens like the Tecnis Multifocal IOL.

The Tecnis Multifocal IOL because its unique aspheric design based on wavefront technology provides negative spherical aberration which actually compensates the positive spherical aberration of the cornea and results in a crisp, high contrast far and near vision in all light conditions.

Advanced Medical Optics Inc, USA, is one of the global leaders in eye care and with its continuous R&D, the company has been introducing newer technologies in the field of ophthalmology. The first foldable multifocal IOL to be USFDA approved was from AMO, since then many newer generation lenses have been launched by AMO like ReZoom and Tecnis Multifocal IOL.

The TECNIS multifocal lens incorporates placing of diffractive concentric rings on the optical surface of the lens. This breaks up light falling on each ring into two parts, one focused for distance vision and the other one focused for near vision. Coupled with the technology that corrects optical imperfections (aberrations) in the eye, which develops with age, the lens gives a superior focus of light on retina. Technically this is called improved contrast sensitivity and in layman terms it means a better and sharper vision in low light conditions. Hence the TECNIS multifocal IOL offers excellent distance as well near vision across all light conditions.

The REZOOM multifocal lens is based on the refractive platform and has refractive zones of different sizes. These zones act as different prisms of varying thickness. When light falls on these zones it breaks up in three parts providing the patients, distance, intermediate and near vision. This ability of the REZOOM lens in the focusing light at all distances required for doing day to day activities eliminates the need for glasses or at least reduces dependence on them.

Delhi also has kept pace with the advancements in ophthalmology and some of the top surgeons in Delhi like Dr. Noshir Shroff of Shroff Eye Centre, Dr. Mahipal Singh Sachdev of Centre for Sight, Dr. Sanjay Choudhary and others are active proponents of the refractive trends in cataract surgery.

Dr shroff reaffirmed that the combination of a refractive and a diffractive multifocal can bring in an exciting change in the earlier concept of bilateral implantation of same type of multifocal/ bifocal IOL.

The combination of ReZoom and Tecnis MF resulted in fantastic results marked by 100% independence from glasses in all the patients where it was tried out and the fastest reading speed which was a measure of the better functional vision. The photopic effects were also negligible resulting in very high patient satisfaction.

According to Dr. Mahipal Singh, “a very small population of cataract patients (5%), the pre-existing structure of eye produces some refractive errors like cylindrical number, which are not correctible by these lenses. Therefore a person may feel more comfortable with the use of the appropriate cylindrical powers occasionally in order to get sharper vision”. This according to the surgeons can be corrected with the help of wavefront guided LASIK to remove the need of any glasses. The combination of LASIK with IOL implantation, commonly termed as Bioptics is also getting popular day by day.
